If you received an emailed Annual Practice Fee invoice, pleases pay via the secure embedded link on your emailed statement.

If you received a mailed, paper Annual Practice Fee invoice, please make payment in any of the ways below.

NOTE: The Annual Practice Fee is NOT billable to your insurance. This page is ONLY for the payment of the Annual Practice Fee. To pay your blue statements related to healthcare services billed to insurance (self-pay balances, copays, coinsurance, deductibles, non-covered vaccines, etc.), click HERE

Ways to Pay

 

Pay by Credit Card

In the “Contact Details” section, please include your full name AND invoice number in the field labeled “Full Name” (e.g., Cooper Smith, Invoice #12345). You can find your invoice number next to your name on the address of this invoice. If you do not enter this number, you might be contacted to confirm your payment.

Credit card payments may also be made by calling the billing office at (510) 527-4682 or your doctor’s office at (415) 752-0100 to pay by phone.

Pay by Check
