If you received an emailed Annual Practice Fee invoice, pleases pay via the secure embedded link on your emailed statement.

If you received a mailed, paper Annual Practice Fee invoice, please make payment in any of the ways below.

NOTE: The Annual Practice Fee is NOT billable to your insurance. This page is ONLY for the payment of the Annual Practice Fee. Pay by Credit Card

Credit card payments may also be made by calling the billing office at (510) 527-4682 or your doctor’s office at (415) 752-0100 to pay by phone.

Please include the Patient’s FULL name and invoice number (in the upper right hand corner of your invoice) in the “Contact Details” to ensure that your payment is credited correctly.

Pay by Check
